RiverWoods Manchester is seeking a Full-Time 32 hour a week LNA for a M, W-F (No work on Tuesdays) role to support our Wellness clinic and the needs or our Independent Living Residents! Hours can vary slightly based however typically would be between 8a and 4:30p with no nights or weekends. Staff provide homemaker and personal care services for the residents of RiverWoods. Perform various basic and specialized forms of activities as well as attending to the personal needs and comfort of residents as assigned by the Enhanced Living Home Health Care program. Attend offsite appointments with residents 2 or more days a week.

Under the general direction of the Nurse Practitioner and within established policies and procedures, the incumbent performs the following functions:
Essential Functions
Completes assigned daily resident home health care services tasks as indicated on the Catered Living schedule and as agreed upon by the client and the Director of Social Services and Home Health.
Documents daily services provided to Catered Living clients in the clients Catered Living chart.
Reports and documents observable changes in the residents’ functional, cognitive, daily abilities, behaviors, sleeping patterns.
Reports immediately and documents unusual incidents to the Director of Social Services and Home Health.
Recognizes and respects residents need for privacy, displays sensitivity to residents who need to express fear, concern, and control on their environment and respects the dignity of residents while performing necessary care.
Primary Functions
Assists residents with all activities of daily living.
Checks vital signs as indicated by the residents Catered Living record.
Assists residents with homemaker services such as, but not limited to, laundry, changing bed linens, escorts, companionship, food preparation, shopping, cleaning, etc.
Maintains cleanliness and orderliness of resident apartments.
Recognizes and takes steps to remedy safety hazards.
Active NH LNA License
Prior experience working directly with senior citizens is strongly preferred
Valid NH Drivers License with the ability to pass DMV Check
